Trial Outcome: 20% Functional Cure Rate in B-Well Studies
Two Phase 3 trials, known as B-Well 1 and B-Well 2, have shown that a 24-week course of the investigational antisense oligonucleotide bepirovirsen produced a sustained functional cure in roughly one in five adults with chronic hepatitis B (HBV). The primary endpoint was loss of hepatitis B surface antigen (HBsAg) and undetectable viral DNA for at least 24 weeks after stopping all treatment—an outcome none of the placebo recipients achieved.
The studies enrolled more than 1,800 non-cirrhotic patients across 29 countries between 2022 and 2025. At the primary assessment 72 weeks after baseline, approximately 20% of the bepirovirsen group met the functional cure criteria, compared with zero in the placebo arm. The response was even stronger in patients with lower starting HBsAg levels: about 26% of those with HBsAg ≤1,000 IU/ml achieved a functional cure. Bepirovirsen’s mechanism—reducing HBsAg production to let the immune system regain control—is fundamentally different from standard antivirals that merely suppress viral replication.
Adverse events were more common with bepirovirsen than placebo, though most were mild to moderate. Additionally, the trials excluded patients with cirrhosis and those with very high HBsAg levels, meaning the results cannot yet be generalized to all chronic HBV patients.

Researchers are planning studies that combine bepirovirsen with immune-based therapies—such as therapeutic vaccines or checkpoint modulators—to boost response rates. The Phase 3 success gives these programs a clear benchmark. On the regulatory side, a 20% functional cure rate in a large trial is novel but not overwhelmingly high; the developer is likely to need at least one more confirmatory trial or combination data before seeking initial approval.
