A Fatal AH-64E Crash Near Fort Hood
Two U.S. Army soldiers were killed Wednesday afternoon when their AH-64E Apache attack helicopter crashed near Soldado, Texas, close to Fort Hood. Reports of the two-seat helicopter going down came in around 1:34 p.m. local time, and both crew members were pronounced dead at 2:30 p.m., according to the Army account.
The deceased service members were being taken to the Carl R. Darnall Army Medical Center on the base, and their names were being withheld until families could be notified. Brig. Gen. Ethan Diven, acting commanding general of the 1st Cavalry Division, said the unit's thoughts and deepest sympathies were with the families, friends and fellow troopers involved.
Although the crash sparked a brush fire that covered about 155 acres, Bell County officials confirmed that no civilian buildings or infrastructure were damaged. The fire was largely contained by evening. The cause of the crash is under investigation, and the Army has not yet indicated whether mechanical failure, human error, weather or another factor was responsible.

Over a four-year period, the Apache fleet logged 17 serious incidents, far outpacing other rotary-wing platforms such as the UH-60 Black Hawk, with a serious incident rate of 4.41 per 100,000 flight hours.
Those figures do not explain Wednesday's crash, but they explain why the platform is already a central focus of Army aviation safety. The incident is the first fatal AH-64 crash of the year, but it arrives after a series of fatal training accidents that have kept the fleet under scrutiny.
Army data point to human error more often than mechanical failure
U.S. Army safety data cited in the report indicate that the vast majority of major Apache mishaps are attributed to human errors rather than mechanical failure. That pattern shifts the likely line of inquiry toward pilot decision-making, crew coordination, training conditions and operational tempo — not just the condition of the aircraft.
Still, the specific cause of the Soldado crash has not been released. Any conclusion about human error in this incident would be premature until the Army's investigation examines flight data, maintenance records, weather and witness reports.
Fort Wainwright, Booneville and the strain of training operations
Wednesday's crash follows two high-profile fatal training incidents. In April 2023, two Apaches from the 1st Attack Battalion, 25th Aviation Regiment collided near Healy, Alaska, killing three soldiers and injuring a fourth; that collision prompted an Army-wide aviation safety stand-down. In February 2024, a Mississippi Army National Guard Apache crashed near Booneville during routine training, killing both pilots.
The fact that the earlier incidents occurred during training may focus attention on the conditions under which Apache crews practice complex missions — including low-altitude flight, night operations and formation flying — although no such conditions have been confirmed in the Texas crash.
What Army Aviation Will Be Asked Next
For Army aviation leaders, the immediate issue is the formal safety investigation, not fleet-wide conclusions. The practical questions are narrow and tied to this event:
- Army safety investigators' preliminary findings. The formal inquiry will determine whether the AH-64E crash stemmed from mechanical failure, human factors, weather or another cause; its conclusions may determine whether a targeted safety action — such as a stand-down or revised training procedure — follows.
- Congressional and DoD oversight interest. Given that Apaches accounted for nine of 17 Class A mishaps in fiscal 2024, defense committees and Army aviation leaders are likely to ask what changed after the 2023 stand-down and how AH-64E training and crew-resource management are being adjusted.
- Unit-level review of training tempo and records. Nearby AH-64E units can use the crash as a trigger to review maintenance and flight records, recent training profiles and crew rest cycles — not because the cause is known, but because the platform's own safety data already flags human performance as the dominant risk.
