The Hidden Dangers of Neck Cracking

The satisfying pop you hear when you twist your neck is usually harmless – but neurologist Alexander Tkachev warns that deliberately forcing that loud crack can, in rare cases, tear the inner lining of a major artery supplying the brain.

The vertebral arteries run through the neck and deliver blood to the rear parts of the brain. A sharp, extreme rotation or bend can cause what doctors call a vertebral artery dissection: a small tear in the vessel wall. On that injury a blood clot can form, potentially blocking flow and triggering an ischemic stroke. “Such complications are rare,” Tkachev told Russian newspaper Izvestia, “and most people who crack their neck occasionally never face them.” However, the risk rises for those with underlying vascular disease, congenital connective-tissue weakness, previous neck trauma, or when the movement is especially rough.

Red flags after a sudden neck twist include severe or unusual pain in the neck or back of the head, dizziness, loss of coordination, double vision, numbness in the limbs, or weakness. Tkachev stressed that the urge to “stretch” the neck usually comes from muscle tension, poor posture, a sedentary lifestyle or spinal issues – not from misaligned vertebrae. While the crack brings fleeting relief, it does nothing to address the root cause.

To prevent the problem, he advises staying physically active, doing exercises that target the deep neck muscles, setting up an ergonomic workspace, and treating any underlying condition. A one-off, accidental crack is no cause for alarm, but turning violent neck twists into a daily habit is best avoided.

When a Stiff Neck Becomes a Vascular Risk

The anatomy explains both the sound and the danger. Most neck cracks come from gas bubbles forming and collapsing in the joint fluid, or from ligaments snapping over bone. That mechanism is benign. However, the vertebral artery sits in a bony canal within the cervical vertebrae. A rapid, forced rotation can stretch or shear the artery, especially if the movement goes beyond normal range. In a small number of cases – often documented in the medical literature – this has led to dissection and stroke even in young, otherwise healthy adults.

Because the event is so rare, no precise population-wide risk exists, but neurologists see a pattern: patients who habitually twist their necks with force or rely on aggressive self-manipulation are the ones who occasionally turn up in emergency rooms with dissection-related symptoms. Those with conditions like Ehlers-Danlos syndrome, Marfan syndrome or a history of vascular disease are more vulnerable, but any repetitive trauma matters.

Why Cracking Doesn’t Solve the Underlying Problem

Tkachev’s main point is that chronic neck tightness rarely stems from a bone out of place. It is almost always a muscular issue driven by static postures – hours at a desk, looking down at a phone, stress. The pop might briefly stretch the joint capsule and relax surrounding muscles via a neurological reflex, but it does nothing to correct the muscular imbalance. Over time, relying on cracking can even destabilize the neck, making people feel the need to do it more often.

The safer road is to strengthen the deep flexor muscles that support the cervical spine and to move regularly. As Maxim Sutormin, chief neurologist for the Moscow Region, told the same outlet, physical inactivity is the second-biggest risk factor for stroke after hypertension. He recommends at least 10,000 steps a day and swimming one or two times a week – beneficial not just for blood vessels but for the entire musculoskeletal system.

When to Worry and How to Keep Your Neck Healthy

  • Avoid deliberate, forceful neck cracking. A gentle, unforced stretch is fine, but never twist your head violently trying to hear a pop.
  • Address the real cause of stiffness. Target prolonged sitting and poor posture. Take frequent breaks, adjust your screen to eye level, and do chin-tuck exercises to activate the deep neck muscles.
  • Strengthen and move. Daily walking (aim for 10,000 steps) and swimming one or two times weekly can reduce both muscle tension and overall stroke risk.
  • Know the emergency signs. If after a neck movement you experience severe neck pain, dizziness, double vision, coordination problems, numbness or weakness, seek immediate medical help – these can signal an arterial tear.
  • If you have a known vascular condition or connective-tissue disorder, be especially cautious. Discuss any neck manipulation – even from a professional – with your doctor first.
